TERMS & CONDITIONS
These Terms and Conditions form a client agreement between you the client (hereby referred as you, your) and Adelaide Breastfeeding Support, ABN 80831271260 (also referred to as Jennifer Gillett).
These Terms and Conditions also include the Fee Schedule, which you must read along with these terms and conditions.
In making a booking with Adelaide Breastfeeding Support you are accepting these Terms and Conditions and Fee Schedule.
DESCRIPTION OF SERVICE
These Terms and Conditions relate to services provided by Adelaide Breastfeeding Support which include; antenatal and postnatal parenting support; infant feeding including but not limited to breastfeeding, chest feeding etc; diagnostics and supply of prescription items where required and within the scope of practice of an Endorsed Midwife; lactation support and management via in person consultations, telehealth consults and written material.
By booking your appointment by submitting the Intake Form you are accepting Adelaide Breastfeeding Support’s Cancellation and ‘No Show’ policy and Fee Schedule as outlined in these Terms and Conditions.
FEE SCHEDULE AND PAYMENT

You accept the Fee Schedule as it pertains to the relevant services you have booked.
Payment is expected on or before the day of your appointment.
Payment for services can be made via cash, bank transfer, debit or credit card via a third party payment system (Square).
24 hours notice is required to cancel or reschedule your appointment and not incur additional costs.
If you cancel your booking within these 24 hours, but no less than 12 hours and do not attempt to reschedule your appointment within 48 hours from your original appointment time, 50% of your appointment fee will be charged.
If you cancel within 12 hours of your appointment time and do not attempt to reschedule your appointment within 48 hours of your original appointment, 100% of the appointment fee will be charged.
New appointment times are subject to availability.
If your appointment is cancelled by Adelaide Breastfeeding Support within 24 hours of your appointment time, no financial penalty will be incurred by you.
If in the event your appointment is cancelled by Adelaide Breastfeeding Support and no suitable replacement appointment can be arranged in a reasonable timeframe, your full deposit will be refunded.
A “No Show” occurs in the event that Adelaide Breastfeeding Support arrives at the designated appointment address at the appointment time and you are not there within 15 minutes of the appointment commencing.
In the event of a ‘no show’ all reasonable attempts will be made by Adelaide Breastfeeding Support to contact you at this time.
If you cannot attend the original appointment 50% of the appointment fee plus travel costs will be charged.

LIMITATION OF LIABILITY
To the extent permitted by law I, Adelaide Breastfeeding Support, will not be liable for any injury, loss, claim, or any direct, indirect, incidental, punitive, special, or consequential damages of any kind, including, without limitation lost profits, lost revenue, lost savings, loss of data, replacement costs, or any similar damages, whether based in contract, tort (including negligence), strict liability or otherwise, arising from your use of any of the service or any products procured using the Service, or for any other claim related in any way to your use of the Service or any product, including, but not limited to, any errors or omissions in any content, or any loss or damage of any kind incurred as a result of the use of the service or any content (or product) posted, transmitted, or otherwise made available via the service, even if advised of their possibility.
DISPUTE RESOLUTION PROCESS
Adelaide Breastfeeding Support are committed to their clients and their customer satisfaction. If, in the event you are unhappy with the Services provided by Adelaide Breastfeeding Support I encourage you to contact me at firstname.lastname@example.org to discuss your complaint and rectify if appropriate.

DISCLOSURE OF YOUR INFORMATION
As an Endorsed Midwife, I have a Collaborative Arrangement with Dr. Cathy Love, and in the event that you do not have a referral from, or agree to liaison with your chosen GP, I may request her collaboration in your and your baby's care plan.
Your care plan may require the involvement of other allied health practitioners - in this instance your consent would be sought before your personal details are shared for collaboration purposes.
I am committed to protecting your personal information, and as such I do my best to ensure your personal information is protected from misuse, loss, interference and unauthorised access, modification or disclosure. For example, whenever I ask for your financial details online, I use security-encrypted response forms and your information is held in secure servers.
However, data protection and security measures are never completely secure and, despite the measures I have put in place, I cannot guarantee the security of your information. You must take care to protect your personal information (for example, by protecting any usernames and passwords) and you should notify me as soon as possible of any security breaches. If you do not wish to pay your appointment fee online, please contact me to arrange for an alternative payment method.
